Digital waivers & consent forms for clubs
Paper waivers get lost, consent forms come back half-filled, and the one parent you really needed to sign never does. ClubApp lets your club collect waivers, consent and permission forms and code-of-conduct sign-offs as in-app read-&-sign — members sign on their phone, and the committee gets proof and an audit trail. Here's how it works.
Say goodbye to the paper pile
Instead of printing forms and collecting them at the gate, you upload the waiver, consent form or policy into your club's document library in the dashboard and mark it to require a signature. From there, members handle it on their phones — no printing, no scanning, no shoebox of forms to lose. It's all part of ClubApp's broader documents & e-signatures toolkit.
How members sign — right on their phone
A document can require one of two kinds of acknowledgement:
- Read — the member confirms they've read it (good for policies you need people to have seen).
- Read, agree & sign — the member reads it, agrees, and draws a signature on screen with their finger (for waivers and consent forms).
The app notifies the people you've targeted that there's something to sign, so the form comes to them rather than you chasing it. A parent can sign a season waiver from the couch; a member can agree to the code of conduct before their first training run.
Proof and an audit trail for the committee
This is the part that matters when questions come up. Every signature records the member's name, email, a timestamp, the signature image and a full audit trail. From the dashboard, the committee can see who has and hasn't signed each form — so you can tell at a glance whether every player has a waiver on file before round one.
No more wondering whether a form was actually returned. The record shows who signed what, and when — kept in one place rather than scattered across inboxes and filing cabinets.
When a policy changes, force a re-sign
Policies and waivers get updated between seasons. When you revise one, bump its version and everyone is required to re-sign. Earlier sign-offs show as stale, so you always know who has acknowledged the current wording — not a version from two seasons ago. It's a tidy way to roll out an updated code of conduct or refreshed consent form to the whole club.
Targeting juniors' parents and guardians
Junior sport lives and dies on consent. Because you can target specific teams or the whole club, you can send a permission or consent form straight to the right group — and with parent & guardian accounts, the adults responsible for a junior are the ones who read and sign on the child's behalf. That makes ClubApp a natural fit for child-safety and safeguarding paperwork, where it matters that the right adult has seen and agreed to the policy.
Nudge the stragglers in team chat
There's always a handful who haven't signed. You can share a document as a card in team chat with its sign status showing, so the form lands where the team is already talking — a gentle, visible nudge that gets the last few over the line without singling anyone out.
What you can move off paper
Clubs use read-&-sign for waivers, consent and permission forms, codes of conduct, child-safety and safeguarding policies and membership terms & conditions. If it's something people currently sign on paper, it can live in the app instead — faster to collect, impossible to misplace.
